What is the registration procedure for New or Returning Association Members ?

A:

Registration takes place at the pool deck. Assistant Coaches will be on hand to speak with you and your swimmers.

  1. Download (can download forms and flyers bundle or individually) 
  2. Fill Out Forms
  3. Bring Checks
    • 1st  Check: Everyone needs a check for Mavericks Swim Association
    • 2nd Check: New and 1st of Year Returners may need a second check for Pacific Swimming Insurance
  4. Go to pool 
    1. Meet the Coach Staff to discuss or confirm the level and workout time for your swimmer. During the season coaches are available from 6:00 to 6:15 or you can make an appointment by sending an email, click here.
    2. Pick up: US/Pacific Swimming form for each child
    3. Submit (to coach or deck office or Black Mailbox in pool alcove): 
      • Fees Calculator (and Mavericks Check)
      • Swimmer Emergency (returning swimmers must update every session)
      • Code of Conducts
      • Athlete PSA Form (and Pacific Swimming check, if applicable)
      • Liability (optional)
      • Volunteer (optional)

Seasonal Athlete PSA Form
This is the USA/Pacific Swimming Registration and Insurance form. This form is only available at the pool deck. Annual renewal is typically done during the Fall session and is good till December 31st of next year.

Athlete Code of Conduct
Each swimmer is expected to follow the Code of Conduct. Signing means you agree to live up to this code in order to show respect to your teammates, coaches, and the sport.

Expense Reimbursement Form
